Health Departments receive individually tailored services (ICBA) to address specific challenges and needs. Once your request has been assigned to Shared ActionHD, we will contact you to inform you. We will coordinate a call with you to discuss the request and identify the next steps. Through the individual consultation, Shared ActionHD staff work one-on-one with your agency on needs specifically related to the implementation of your programs and target population that you are trying to reach. With your feedback, we will develop an action plan to address the needs expressed in the CBA request. To work with us, submit your request through the CRIS system.
